How to run an OPD token queue at reception

A token queue works when three things are clear to everyone: how tokens are issued, which token is running, and what happens to bookings and late arrivals. Decide those rules once, show the running token where patients can see it, update it every time the doctor calls the next patient, and reset it at the start of each day.

Why use tokens instead of names?

Calling names aloud or writing them on a board tells everyone in the waiting room who is there to see the doctor. A token number does the same job, putting patients in order, without exposing anyone. It is also faster to call and harder to mishear.

Tokens also make the queue measurable. A patient holding token 18 who can see token 12 running knows roughly where they stand, and does not need to ask.

What rules should the clinic decide first?

Write these down and give them to every receptionist, so the queue runs the same way on every shift:

  • How tokens are issued: on arrival at the desk, on booking, or both.
  • Whether booked patients get their own token range, or are fitted into the walk-in sequence at their slot time.
  • What happens if a patient misses their token: they go to the end, or are seen at the next gap.
  • When the token resets: usually at the start of each day, or at the start of each batch if the clinic runs separate morning and evening queues.
  • Who is allowed to change the running token, and how mistakes are corrected.

How do you run the queue through the day?

  1. 1Before the first patient arrives, reset the token and confirm the doctor's status and today's timings.
  2. 2Issue tokens in order as patients arrive, and tell each patient their number.
  3. 3Every time the doctor calls the next patient, move the running token forward straight away, before doing anything else.
  4. 4If the doctor steps away, update the doctor's status so waiting patients know why the token has stopped.
  5. 5Fit booked patients in at their slot time according to your rules, and note any who don't come.
  6. 6At the end of the day, note how many tokens were issued so you can plan staffing for busy days.

The most common failure is a running token that is not updated. If the display falls behind, patients stop trusting it and return to the desk to ask.

How should bookings and walk-ins share one queue?

Pick one approach and explain it to patients. The simplest is to treat a booking as a reserved place in a batch: the booked patient is seen at their slot time, and walk-ins fill the gaps. Whatever you choose, the running token should always show the token the doctor is seeing now, so the display stays truthful for everyone.

What about late arrivals and missed tokens?

Decide a rule, display it at the desk, and apply it the same way every time. A common, fair rule is that a patient who misses their token is seen at the next gap rather than immediately. The rule matters less than applying it consistently. Patients accept a clear rule; they argue with an inconsistent one.

How do patients see the running token?

Options range from a whiteboard to a display screen to a web page. A whiteboard only helps people standing in front of it. A screen helps people in the waiting room. A web page helps patients anywhere, including at home, deciding when to leave.
